SSN and applicant name do not match
Letters were sent by PFD informing applicants that the Internal
Revenue Service (IRS) has notified PFD that either the Social Security
Number (SSN) or the name provided on an application does not match IRS
records. If the discrepancy is not corrected, Federal Law requires that
PFD withhold 28% of the dividend and send it to the IRS.

Correct any discrepancies to the name or Social Security Number
on the letter from PFD and return it as soon as possible.

If the information is correct, contact your nearest Social Security Administration office.

If PFD does not receive the corrected information by June 26, 2009,
you will be required to make any corrections through the
Social Security Administration Office by requesting a SSA-7028 be sent to the PFD Division.

Allow sufficient time for your request to be processed, as withholding cannot be avoided
if your request is received within 30 days of your scheduled payment date.
